What do I have to do for the New patient Paperwork?
- Visit our New Patient Forms, print out the forms, answer all of the questions as detailed as possible. Please bring these completed forms to your first appointment.
- It is important that all forms are completely filled out in their entirety prior to arriving at your first visit, in order to insure that we have the appropriate amount of time to complete your full exam on the date of your visit. Please double-check that all questions are answered and that your signature and the date are entered.
- If you do not have access to a printer, or for some other reason you will not be able to complete the forms before your first visit, please tell our office when you call to make your appointment, and we will schedule you appropriately.
- Our goal is to provide you with the highest quality care possible and the examinations and treatments that we provide require a specific amount of time. We value our patients time and never want to leave our patients waiting. By notifying us about the paperwork in discussion, we can schedule you appropriately
Will I have to go to a chiropractor forever?
The Chiropractic Sports and Rehabilitation Center adheres to the most recently peer reviewed Guidelines for Chiropractic Care. These guidelines were developed by the chiropractic professional associations and used by many in the insurance industry to establish ethical and appropriate protocols for the treatment of chiropractic patients. What this means to the patient is that they will never be subjected to any unnecessary diagnostic tests or treatments, and the length of their treatment will depend on the severity, complicating factors, and characteristics of their condition. Our office prides itself in identifying and providing our patient's specific and individual needs.
Will I need diagnostic testing or x-rays?
Radiological studies will only be requested if they are deemed medically necessary. This means that if something in the patient's past medical history or physical examination that indicates the need for additional diagnostic testing; they will then, and only then, be preformed or sent out to be done at the patient's approval.
Will my insurance cover my care?
Dr. Rykaczewski is a participating provider with most medical insurance plans, secondary insurance coverage, auto insurance carriers, Medicare, union plans, and third-party administrator plans. Please contact our office for specific questions about your insurance: 856-874-0080
