- ...that in order to expedite your initial visit that all new patients would have these forms completed prior to their first appointment. If not completed, please inform our office upon scheduling your first appointment.
- It is important that all forms are completely filled out in their entirety prior to arriving at your first visit, in order to insure that we have the appropriate amount of time to complete your full exam on the date of your visit. Please double-check that all questions are answered and that your signature and the date are entered.
- If you do not have access to a printer, or for some other reason you will not be able to complete the forms before your first visit, please tell our office when you call to make your appointment, and we will schedule you appropriately.
- Our goal is to provide you with the highest quality care possible and the examinations and treatments that we provide require a specific amount of time. We value our patients time and never want to leave our patients waiting. By notifying us about the paperwork in discussion, we can schedule you appropriately
